Public Safety Vacation Watch Request

This form is used to notify the PID 6 Patrol Officers, FWPD, of your upcoming vacation. The officers cannot guarantee that they will be able to check your residence at specific times or dates, but they will however make every effort to make as many checks on your residence as time and workload permit.

Please remember to contact Lt. Grady at 817-994-3834 if you return early from your vacation or if your departure or return times change.

Complete the on-line form to send directly to the PID 6 Patrol –OR– you may prefer to use an alternate format and send it via your own e-mail software:

Offline Form Instructions:

  1. Right-click on one of the links above. Click Save Target As.
  2. Save it to a place on your PC.
  3. Open the file and fill it out with as much complete information as possible.
  4. Save the changes made.
  5. Email the completed form to pid6patrol@aol.com. The subject line should contain: "Vacation Watch Request for [insert name]". and include the date range you are out.
  6. To email it, attach the file to an email note using your email program.
